Variables and definitions

LODS is calculated within the first 24 hours. Each organ sytem receives points for the single variable associated with the most points.

All variables must be measured at least once. If they are not measured, they are assumed to be within the normal range for scoring purpose. If they are measured more than once in the first 24 h, the most severe value is used in calculating the score.

Neurologic system = GCS : use the lowest value; if the patient is sedated, record the estimated GCS before sedation.

Cardiovascular :

HR : use the worst value in 24 h, either low or hight rate. Assign 5 points ( HR< 30 per minute) for cardiac arrest.

SBP : use the same method as for heart rate.

Renal system :

Serum urea or serum urea nitrogen level : use the highest value.

Urine output : if the patient is in the ICU for less than 24 h, make the calculation for 24 h. If the patient is on hemodialysis, use the pretreatment values.

Pulmonary system : if ventilated or under CPAP, use the lowest value of the PaO2/FiO2 ratio. A patient who has no ventilation or CPAP during the first day is free of pulmonary dysfunction.

Hematologic system: W.B.C. : use the worst (hight or low) count that scores the highest number of points.
